To successfully manage their symptoms, persons who are mentally ill need a comprehensive network of interdisciplinary services, including medication, psychiatric care, counseling, supportive housing, rehabilitation services, and prevocational and vocational services. Pathways sponsors a continuum of care and programs to help the mentally ill fulfill their potential and lead productive lives in the community.
1.INPATIENT/HOSPITAL SERVICES: Pathways works with the discharge team at the hospital to develop a solid plan to facilitate a person's move from the hospital to the community.
2.OUTPATIENT CARE: Pathways arranges for clients to have outpatient psychiatric care including setting up a day program, a psychiatrist, and other supportive services.
3.MEDICATION: Pathways offers medication monitoring at all of its housing facilities.
