Treating Mental
Illness To
successfully manage their symptoms, persons who are mentally
ill need a comprehensive network of interdisciplinary
services, including medication, psychiatric care,
counseling, supportive housing, rehabilitation services, and
prevocational and vocational services. Pathways sponsors a
continuum of care and programs to help the mentally ill
fulfill their potential and lead productive lives in the
community.

INPATIENT/HOSPITAL SERVICES: Pathways works with the
discharge team at the hospital to develop a solid plan
to facilitate a person's move from the hospital to the
community.

OUTPATIENT CARE: Pathways arranges for clients to have
outpatient psychiatric care including setting up a day
program, a psychiatrist, and other supportive services.

MEDICATION: Pathways offers medication monitoring at all
of its housing facilities.

HOUSING: Pathways provides a range
of housing options.

Milbank Community Residence: A licensed group home for 8
residents; targeted for individuals who require 24 hour
supervision.

Davis Supervised Apartment Program: 3 apartments,
capable of housing a total of 8 residents; targeted for
individuals ready for more independent living.

Brookside
Apartments: 8 studio apartments and 4 one bedroom
apartments; designed for individuals who do not need the
structure or degree of supervision offered in Pathways'
other residential programs. Rent is subsidized by HUD's
Section 8 Program, making it possible for tenants to pay
only 30% of their income for rent.

509 East Putnam
Avenue Residence: permanent housing in a congregate
setting for ten individuals. Each resident has a private
room with private bath. Meals, recreation, and support
services are available on site. Residents pay 30% of
monthly income for rent.

RESIDENTIAL SUPPORT SERVICES: Pathways offers outreach and
care management for individuals with mental illness who live
independently in scattered site apartments throughout
the community. Services include supportive counseling,
help with daily living skills (such as cooking,
cleaning, and budgeting), encouragement and/or advice on
dealing with problems that arise when living in the
community.

PSYCHOSOCIAL REHABILITATION SERVICES: Pathways'
Fellowship Club offers individual and group activities
to build daily living skills, cultural and educational
programs, social/recreational activities, and a free
breakfast and lunch program.

PREVOCATIONAL SERVICES: Pathways provides
prevocational training and counseling to help clients
develop good work habits and identify potential career
paths. Clients operate a mail service as part of their
training.
