Milbank Community Residence: A state of CT licensed
group home for 8 residents with 24 hour/7 day a week
supervision. The residents receive intensive training with activities of daily living and an assigned case manager.
Davis Supervised Apartment Program: 3 apartments,
capable of housing a total of 8 residents; targeted for individuals ready for
more independent living. Support staff 7 days per week including a case manager for each resident.
Brookside Apartments: 8 studio
apartments and 4 one bedroom apartments; designed for individuals who do not
need the structure or degree of supervision offered in Pathways’ other
residential programs.This is our independent living program with case management services.
509 East Putnam Residence: 10 single
rooms with private baths; congregate meals and support services available on
site, including case management services.
